The first 33 sick days a year are paid by the employer, after that — by ZUS (14 days for employees aged 50+). For you the payment flow does not change.
How sick pay is calculated
The calculation runs in three steps. First, your average monthly gross salary over the previous 12 months is taken. Then your ZUS contributions are subtracted from it — pension, disability and sickness combined 13,71%. The resulting amount is the “base.” It's divided by 30 and multiplied by the rate: 80% in ordinary cases or 100% in special ones.
That's why sick pay is always lower than your normal salary — and why a long illness noticeably hits your budget. You can work out the difference above.

illness during pregnancy;
incapacity due to a workplace accident or an accident commuting to/from work;
incapacity due to donating blood or tissue, and related examinations.
FAQ
From which day of work can you take sick leave?
The right kicks in after a waiting period: usually 30 days of continuous mandatory insurance (umowa o pracę), or 90 days for voluntary cover (zlecenie). There are exceptions — for instance if you had insurance earlier without a long gap.
Can you work while on sick leave?
No. ZUS may carry out checks during sick leave; working during that period can forfeit your right to payment.
What if I'm on an umowa zlecenia?
Sick pay is only paid if you were paying the voluntary sickness contribution (2.45%). Without it, there's no payment. That's the main hidden difference between contract types.
